The Certificate of Completion is where you will provide information on a project. It will need to be completed and signed by you, the Contractor. There are several sections to the Certificate of Completion. We will address each of them in this video. There is a box marked “To be completed by PCI or PFL” at the top of the Certificate of Completion. Leave this section blank for the lender to fill out. Section 1 is where you will enter your Qualified Contractor Business Name and CSL number. List your name as you provided it on your Application to the Program. Section 2 is where you will enter information specific to this project. Start by entering the project start date. Next, enter any permit numbers for permits obtained while completing this project. If no permit was necessary, check the box marked quot;Not Applicable.” If a permit is required for your project, you must provide the permit number. Next, you will enter the project's address, as well as the City and ZIP code for the project. Note that the project address should match the service address on the Customer×39’s utility bill. Now you have to evaluate the project scope to determine if the project included 3 or more EEEMsincluding at least one of the measures described here:whole building air sealing, duct sealing, and/or duct replacement, or attic insulation and air sealing. If the project you completed does meet these criteria, check the box marked yes here. Make sure to fill out Section 4 if you answered yes to this question. In Section 3, you will enter information for the utilities that service the project address and the rebate incentive information associated with the project. Select the Borrower's gas utility and electric utility. It is important to remember that the Borrower may have gas and electric service provided by different utilities. If the utility is not one of the utilities names specifically on the form, please check the box marked quot;other” and write in the name of the utility. In this case, Ellen gets both her gas and electric from Stamp;E. Make sure to enter both the gas and electric providers and any applicable rebates or incentive information for the project. If it is available at the time you are completing the Certificate of Completion, please include the rebate or incentive ID number. Be sure to enter the rebate or incentive amount and to clarify if that is an actual dollar amount or an anticipated dollar amount. Finally, check the box to clarify if the rebate or incentive will be directly applied to the cost of the project. Section 4 is where you enter information on the Combustion Appliance Safety Test. You only need to fill out this section if you answered quot;yes” to Question (h) in Section 2.
